Position Qualifications:

  • Certification in a related field preferred (Unit clerk, medical reception, medical office assistant, office administration, etc)
  • Current CPR certificate preferred
  • Experience in the health care field an asset
  • Superb proficiency in the use of computers & office equipment a MUST (especially Microsoft Excel)
  • Above average communication skills, both verbal & writtenStrong multi-tasking skills
  • Impeccable organization skills
  • Ability to maintain patient privacy and confidentiality
  • Good prioritization skills
  • Ability to collaborate with other multi-disciplinary team members
  • Ability to work independentlyProblem-solving skills essential

Position Description:

  • Assist the Clinical Care Coordinator by:
  1. Gathering statistical information for chronic disease / clinical indicator reporting
  2. Gathering statistical information for primary care measurements (supply, demand, activity, etc)
  3. Performing tasks related to chronic disease care management: phone call reminders, assisting in group visits
  4. Creating or editing templates in the Electronic Medical Record (EMR) to assist in proper data collection practices
  5. Helping improve clinic workflow processes to advance primary care practices
  6. Performing any other tasks deemed appropriate
